[Evergreen Bergvliet]

        Dear Resident

As announced, Evergreen Care commenced operations in the beginning of October at Bergvliet, Diep River, Noordhoek and Lake Michelle, bringing our care services in-house, to ensure that we improve service levels going forward. You may have noticed the new look clinic that is being managed by SN Zuko and his 24-hour care team. The Evergreen Care nursing staff will undergo continuous training throughout the year. Just a reminder that Emergency Care should always be summoned via the Telecare Unit in every home and apartment and not via the Clinic Nurse. This will ensure that the ambulance and paramedics are dispatched immediately.


-       Evergreen Care Nursing Personnel on duty:
 1 Enrolled Nursing Auxiliary (ENA) per day and night shift (24/7)
 An Enrolled Nurse would work between the clinics, supervising the ENAs.

-       Care Workers: Competent trained care workers will be appointed to take care of daily living activities for residents.

-       A highly qualified Healthcare Risk and Compliance Manager, Guin Lourens, has been appointed to take responsibility for developing standards, policies and procedures for all the clinics to ensure that we comply with Statuary Regulations as well as COHSASA standards.

-       Evergreen Care has also appointed a Care Manager, previously an experienced Village Manager - Melanie Carstens who is also appointed a qualified Social Worker,  to consult with residents and their families.

The services offered and charged for in terms of Board of Healthcare Funder Rates (BHF) - see charge sheet attached:
 Medical and nursing procedures such as wound care, catheterisation and IV therapy.
 Placement of nurses or care workers for day and night homecare.
 Hospital visits and assistance with post-operative care.
 Health risk assessment and chronic disease management.
 Multi-disciplinary services delivered in the home, which may be charged directly to the residents’ medical aid: Physiotherapy/Occupational Therapy/Speech Therapy/Dietician/ Psychiatrist.
