        Dear Residents

Annual DQ98 Assessment - 2026

In accordance with the requirements of the Older Person's Act, all residents are required to undergo an annual assessment. As part of Evergreen Health's annual healthcare review process, we kindly request that you complete the attached Resident Health Self-Assessment.

Residents may complete and submit the Self-Assessment in one of the following ways:

  *   By completing a printed hard copy and return to Reception
  *   Via the JotForm Self-Assessment link: https://form.jotform.com/Elize_Porter/muizenberg-evergreen-lifestyle-resi

If you would prefer to complete this assessment with EN Sharon Adams on a Thursday, you are welcome to make an appointment with Reception on Ext 4200.

We would like to have all assessments completed and returned by Monday, 31 August 2026.
