        Dear Resident,

In our continued effort to improve our efficiency regarding healthcare, please find attached a page we are asking you to complete.

The primary reason, that in a time of emergency we can quickly access your medical information should you need to go to a hospital or are taken by an ambulance. Any para-medic, ambulance service or hospital needs this information before transporting you and when admitting you.

Print a hard copy of this document together with your:

*           Identity document
*           Medical aid card (front and back)
*           List of all medication

Once completed, leave it in a convenient easily accessible spot in your home. We suggest a bedside cabinet drawer in a sleeve or envelope. Kindly send us have a copy so we are able to update your medical file accordingly.
