Dear Residents,

Not all residents needs Sub-Acute Care and this is mostly determined in hospital, and  of vital importance
that we (HC) visit the patient in hospital on the day of surgery for either planning;

  1.  Transitional care either to:
a)      Home with EH Home Care in place
b)      Muizenberg Sub Acute Care

Muizenberg Sub-Acute Facility

Admission Criteria for Sub-Acute Care

Step 1
Residents report to Evergreen Manager on scheduled upcoming surgery or admission to hospital
Manager inform Healthcare team and NSM Muizenberg

Step 2
The HC Team would visit the patient (resident) in hospital
Arrange transition to Muizenberg Care Centre with Dr as well as Hospital Case Manager, if required
EH case managers would contact medical aid for authorization together with the Dr's motivational letter

Step 3
Patient admission for Short term Care to Muizenberg Care Centre
Patient is treated and transition to home is planned on day 1 of admission.
Home Care Services would be arranged already in the Care Centre.
