        Dear Resident

We are looking forward to welcoming you to the Evergreen Lifestyle Village at Val de Vie.

In keeping with welcoming of residents to the Evergreen Lifestyle ‘family’ we need to ensure we are prepared to meet your needs.

Attached please find:

•         A Medical Certificate questionnaire that must be completed by your personal medical practitioner. Please do note that we need a completed certificate for each resident in your home.

Following this letter, we will be in telephonic communication shortly, confirming that you have indeed received this note and the documents required.

•         In order for us to keep the process flowing smoothly, we kindly request that you submit your completed medical certificate questionnaire by no later than Monday 20 May 2019.

•         You are welcome to scan a copy and return it to atties at evergreenlifestyle.co.za<mailto:atties at evergreenlifestyle.co.za> or alternatively you may leave a printed copy for us at the reception desk at “The Yard” in Val de Vie.

•         During the final week of May, we will call you to set up a time, convenient to you to conduct a medical interview with our village health team. This interview should not take longer than one hour. The timings for these interviews will take place during Monday 3 – 24 June 2019.

Importantly, the interview has absolutely no bearing to your acceptability to occupy your home at Evergreen at Val de Vie, it is merely a basis for us to get to know you better and be prepared to meet your needs.

We would also like to introduce you to the village management team:

Village Manager
Attie Shultz, appointed village manager comes with a wealth of hospitality experience, some 20 years, both as an executive chef and a divisional hotel manager.

Assistant Village Manager
Tiaan van der Spuy, comes with a wealth of hospitality experience, working in some of Cape Town’s most prestigious hotels.

We wish them all the best at Evergreen Val de Vie and are confident they will make you feel welcome and content.
