[Evergreen Bergvliet] DQ98 Self-Assessments

        Dear Residents,
In accordance with the Older Person's Act, all Residents are required to complete an annual assessment.  We have decided to allow residents to have the option of doing this as a self-assessment going forward.
If you prefer that the Nurse does this assessment with you in person, please feel free to make an appointment with Sister Sharon via by sending me an email to request this.
We are aiming to have these complete by 30 November 2024, thus for those who prefer to do the self-assessment, please return your completed assessment to me directly.
