In our continued effort to improve our efficiency regarding healthcare,
please find attached a 

page we asking you to complete.

The primary reason, that in a time of emergency we can quickly access your
medical information 

should you need to go to a hospital or are taken by an ambulance.

Any para-medic, ambulance service or hospital needs this information before
transporting you 

and when admitting you. 

Print a hard copy of this document together with your,

*	Identity document
*	Medical aid card (front and back)
*	List of all medication

and leave it in a convenient easily accessible spot in your home. We suggest
behind the front door 

in a sleeve or envelope.

Once completed also let us have a copy so we are able to updated your
medical file accordingly.

A gentle reminder for Wednesdays Evergreen Health presentation at the large
TV area at 2.30 pm
