[Evergreen Muizenberg]

        Dear Resident,

We have over the years had a number of residents questioning why the need to take the
DQ98 assessment. In short, it is mandated and law as far as the older Persons Act of 2006.

Evergreen Health has compiled an extensive explanatory document on why, what, how, who
and when we conduct these health assessments.

Attached and below please find this explanatory document compiled by our Evergreen nursing
manager, Dr Guin Lourens.

Thank you to all the residents who have already assisted us with making time available for their
annual review, where necessary our nurses will be in touch with residents to schedule the
assessment.
