        Dear Resident,

As you might be aware the Department of Health has announced the second phase of the vaccination roll out. The Electronic Vaccination Data System (EVDS) is now open for registration for those who are 60 years and older (see link below)

Find attached two documents; one for those who are able please complete. For those who need help please contact a next-of-kin or your reception.
Once completed please hand in to us at reception, so we can register on your behalf; we need all completed forms back even if you are not having the vaccine.
Any resident who does not have access to a printer, please collect a form from reception.

At this stage we will register forms as we receive them; and you will receive an SMS to confirm the registration has been successful.

The deadline for all online registrations is Monday 17 May 2021, however the Department of Health may adjust this date.

Also attached you will find communication from Dylan Pienaar, Evergreen Financial Director on the new Evergreen Health pricing structure.
