Dear Residents,

Emergency Record of Resident.

Please find attached emergency records of residents.

In the event of a medical emergencies,

We require the attached form to be completed, the completed form to be placed behind the front door of your home.

This will enable the first responder to have  access  to  vital medical information to provide to the medical professionals.
Some notes on why this form is useful:
1.            Faster Response Times: Having access to residents' emergency numbers enables ambulance services to reach out to designated contacts quickly. This can be crucial in situations where time is of the essence, such as severe medical conditions or accidents.
2.            Medical History and Special Needs: Residents might have specific medical conditions, allergies, or special needs that paramedics should be aware of. Having this information readily available can help medical professionals provide appropriate care and treatment.
3.            Language Barriers: In some cases, residents might not be able to communicate effectively due to strokes. Having emergency contact information can help first responders reach someone who can provide vital information about the individual's condition.
4.            Efficient Communication: Ambulance services can communicate with designated contacts to gather essential information about the resident's health status and medical history, ensuring they receive the best possible care.
5.            Coordination with Healthcare Providers: If a resident has a specific healthcare provider, having their emergency contact information can help in coordinating care and sharing important medical details with the hospital or clinic.

You are by no means forced to have this in place and you are also welcome to omit some information if you feel it infringes your POPI Act rights.

All we ask, is that IF you decide to complete this form, it is left in an accessible area that is "visible" to first responders, i.e., behind your door or on your fridge door etc. as no one will go through your belongings hoping to find such a document if it is not visible.
