Re: Muizenberg Sub-Acute Facility

As of the 6th April 2022 evergreen Muizenberg Care Centre is registered as an 8 bed Private Sub Acute Facility with The Board of Healthcare Funders of South Africa (The organization concerned with the issuing of Practice Numbers)

This Practice Number was issued after Muizenberg Care Centre was awarded a Sub-Acute License by The National Department of Health in November 2021.

Sub-Acute Care is an in-patient programme for patients who have suffered an Acute event as a result of an illness, injury or disease, have a determined course of treatment and who do not require intensive diagnostic and/or invasive medical procedures.

Our Muizenberg Care Centre Sub-Acute practice number enables us to gain authorization for admission from your Medical Aid resulting in payment by the medical aid.

Muizenberg Sub-Acute Facility provides personal nursing care on a Multidisciplinary level which includes the following:

General Practitioner,  Registered Nurse, Case Manager, Physiotherapist, Speech therapist, Occupational Therapist, Dietician and Social Workers

These professionals come together frequently as a team to discuss each individual case and conducts meetings with the family.

The team also works closely with the Health Insurer with individualized treatment plans, preventing re-admission to hospital and planning discharge to your home.

Types of Care you can expect from Muizenberg Sub-Acute Care:

  *   Post-Surgical Care
  *   Intravenous Therapy
  *   Wound Care
  *   Pain Management
  *   Pulmonary or Breathing Care, such as Ventilator or Tracheostomy Care
  *   Cardiac (Heart)Care
When in need of Sub-Acute Care:

It is important to contact your Village Manager, informing them before you go to hospital and what date you will be discharged, as discharge planning and transferring to Sub- acute care pre-authorization planning already starts in hospital.

Is Sub-Acute Care and Frail care the same thing?

No its not. Medical Aid will not cover any expenses for Frail care admission. Sub-Acute admission is post hospital, short term care, that needs pre-authorization from your medical aid.

We urge residents to make use of these facilities that are now provided before utilizing other Sub Acute facilities in the area.
