Dear Residents

Please find attached the Wellness Day poster for Discovery Vitality Wellness Day held at the Lifestyle Centre on Saturday 25 February 2023 from 07:00 – 13:00.
These services are charged for but you can claim them back from your medical aid. (See the letter explaining the offering)

Residents, family, and friends are all welcome to make bookings.

We encourage participants to complete all three assessments because it allows them to earn the most Vitality points; however, if someone just wants to see one
professional for example, the DisChem nurse for a health check, single slots can also be booked.
The booking sheet works in a step format, so if someone sees the Nurse at 7 a.m., they will see the Dietitian at 7:30 a.m., and the Biokineticist at 8 a.m.
Bookings can be made with Tshwanelo at reception during office hours.

Please see the attached form for a more detailed description of the day; once we have received the bookings, we will send through the necessary forms to complete in advance. Please keep in mind that, in addition to the booking time, the patient's name, email, and phone number should be recorded in order to send the necessary documents and statements for reimbursement purposes later on.
