        Dear Residents,

Attached is a copy of the vaccination registration form that needs to be completed by yourself before arriving for vaccination tomorrow.
I am in the process of printing them and will send the Duty Manager around to drop at each house.

Please complete this form and put tomorrow's date on it as your vaccination will take place tomorrow.

Please bring this document with you when making your way to the Life Style Centre tomorrow.
We will have a few extra forms printed and ready at the reception desk.

Having this completed prior will help us speed up the process so that we can get as many vaccinations done as possible tomorrow.
