Dear Residents,
COVID-19 Vaccination for Residents

As you might be aware we have been trying to get registered as a vaccination site for our residents when phase 2 vaccinations are rolled out.
The Dept of Health has asked us to please load all our residents on the Electronic Vaccination Data System (EVDS) by Friday the 16th of April 2021 as part of our application.

We would therefor like to request that you all complete the attached document and get this back to us at the Club house so we can capture it on your behalf.
Printed documents will be dropped off at all your homes tonight for completion and signature.  Please complete in full.

You will notice there are two documents attached.  The first is for Power of Attorney residents who will need their power of Attorney person to complete and sign.  The Second is for able self-residents.

At this stage we will capture forms as we receive them.  We will keep you informed of the following steps as we receive them from the Dept. Of Health.
