Dear Residents,

ANNUAL DQ98 MEDICAL ASSESSMENT BY UNIQUE HEALTH

This service is done by our healthcare service provider, Unique Health and legislated by the Department of
Social Development under the Older Person's Act of 2006. Unique Health will be commencing with the
assessments in January.

The purpose is to establish if there has been any change in your Medical status in the last year and will
also equip us, Unique Health and yourselves to identify and mitigate any health risks and be better
prepared in the event of anything happening.

The assessments will be done by Sr. Marius by appointment only.

Sipho Makhutla, the Ward Secretary, will be contacting all Residents by telephone to make  appointments.
