Evergreen Broadacres Resident's Fun Walk to be held on 22 October from 4.30pm.

All residents walking in the Resident's Fun Walk on Saturday 22 Oct are required to fill in the above Indemnity form per instructions from Amdec
If residents are unable to print this form please contact Ernie, Unit 80 ext 180, and he will give you a copy.

Please return this form to me or to the office
as soon as possible to avoid wasting time at the start of the Walk?
