<p class="MsoNormal">As you might be aware the Department of Health has announced Phase 2 of the vaccination roll out. The Electronic Vaccination Data System (EVDS) is open for registration and request that all residents that are able to do so please register
as part of the application. We will assist all residents that require help in this regard.<o:p></o:p></p>

<p class="MsoNormal">We would like to request that you all complete the attached document and get this back to us at the Club house, we need all completed forms even if you are not taking the vaccine.<o:p></o:p></p>

<p class="MsoNormal">Residents who have partners who cannot complete their own forms and who will need power of attorney consent can let us know and we will forward/print the additional paperwork required in those instances.
